Care Managers

A geriatric care manager is a professional who has training in gerontology, social work, nursing, or counseling. They are able to assist families in assessing their individual needs and arranging services in-home and/or in the community. Care Managers are able to review legal, financial, and medical documents and make referrals to appropriate people in the community. Care managers can also assist in long distance care taking so that family members who live a distance away can be updated or informed of daily living as well as emergency situations. Geriatric care managers also provide counseling and education and are knowledgeable about the various community resources to best assist your family.
